Having four years of experience in the danish hospital pharmacy setting has strengthened my overall clinical skill in medication reconciliation, medication review, clinical research, supervision, drug service, and dispensing. I am an advocate of a structured and targeted approach and an encourager of collaborative and supportive team spirit to multidisciplinary elevate patient care.

Academic focus: Pathology, rational pharmacotherapy, evidence-based drug use, biostatistics, patient communication, medication review, and reconciliation.

Master’s Thesis: Medication review and check of inhalation technique on COPD patients with exacerbations hospitalized in the Emergency Department; An interventional feasibility study.

Clinical Pharmacist at Hospital Pharmacy of Fyn, Denmark

All the below duties are carried out in the Department of Internal Medicine, Gastrointestinal Surgery, and Emergency Department:

– Medication reconciliation and review: Assessing, evaluating, and designing comprehensive drug therapy plans through an evidence-based and multidisciplinary approach for over 3000 hospitalized patients.

– Responsible for inquiries regarding the Respiratory System in the Region of Southern Denmark, including COPD, asthma, and check of inhalations technique.

– Medication counseling at hospitalization and discharge.

– Discharge summaries of changes in the patient medicine list to help physicians with patient discharge and eliminate inappropriate drug prescriptions and interactions.

– Drug service and dispensing to 25 patients daily to ensure medication availability and patient safety.

– Educate 45 physicians and chief physicians in drug-related subjects twice a week and weekly sparring with 16 team pharmacists.

– Responsible for multiple educational activities, including a 6-hours long course on inhalation medicine for COPD and asthma for pharmacy technicians from all hospital pharmacies in Denmark.

– Attend educational activities by physicians and nurses on medical subjects, including antibiotics, diabetes, cardiology, and ulcerative diseases to stay up to date clinically.

– Responsible for educating and training new pharmacists in medication review, check of inhalation technique, and supervising quality projects and audits.

– Responsible for the education of 35 nurses in the Department of Gastrointestinal Surgery in anticoagulants, obstipation, and other relevant subjects in pharmacology over 3 months.

– Ensuring patient safety and drug therapy evaluations by updating the regional guidelines of Southern Denmark regarding the optimal treatment of COPD and asthma.

– Monthly sparring with physicians, nurses, and dietitians about complex hospitalized patients in the Department of Gastrointestinal Surgery to improve drug treatment and reduce hospitalization time.

– Implementing quality projects and audits, including a collaboration with clinical nursing specialists and chief physicians in endocrinology about optimizing rapid insulin treatment in 2019 and 2021.
